At our general dentistry office, we make it easy for new patients to get started. All you have to do is call to schedule an appointment, fill out the new patient’s packet, sign the dental record release form, and list any medications you are currently taking. Each form should only take you a couple minutes to complete. If you have any questions about the forms or anything else, please do not hesitate to contact our office. We can address all of your questions and get your appointment scheduled.
